1. Are displaying any symptoms of COVID-19 including fever or chills; cough; shortness of breath or difficulty breathing; fatigue; muscle or body aches; headache; new loss of taste or smell; sore throat; congestion or runny nose; nausea or vomiting; or diarrhea.)
    

2. Have a COVID-19 test pending.
    

3. Have been around someone who tested COVID-19 positive or whose symptoms are suspicious of COVID-19 in the last 14 days.
